Repair Documentation – Repair Action Document

Clause:

The following documents are required to be submitted to Buyer’s AuthorizedProcurement Representative immediately upon shipment:1. Certificate of Conformance or Certificate of Calibration2. Material Condition tag attached to the unit and on the outside of theshipping container (e.g. Serviceable tag DD1574-1, FAA8130-3, applicableEASA form; OR, Unserviceable tag DD1577-1).a. Each Condition tag shall include Seller’s inspector stamp and, asapplicable, the US Government prime or customer contract numberreferenced in the Contract.b. If several items or unit packs are placed in a single shippingcontainer, each item or unit pack must be labeled or tagged.3. As required, copy of the completed:a. DD Form 250, AND/ORb. DD11494. One copy of Bill of Lading (e.g. Seller or Government) OR Airway BillSeller shall provide the documents above with the returned Property as follows:1. One set inside the shipping container2. One set affixed to the outside of the shipping container which must beimmediately visible and shall be protected by water-resistant transparent tapeor envelope.3. Retain one copy for Seller's files.Seller's performance is not complete until the above documents are received by Buyer
